The Dementia 2020 Citizens' Panel for England

Have you, or has somebody close to you, been diagnosed with dementia in the last few years? 

The Department of Health and Social Care want to understand more about people’s experiences of living with dementia and would like to hear from you. Could you spare 10 - 15 minutes to fill in their survey?

The information they collect will play a direct role in planning and improving services for the future. This is the third in a series of surveys and in this questionnaire they focus specifically on the daily lives of people diagnosed with dementia, dementia friendly communities and dementia research.

The Department of Health and Social Care for England has asked an independent organisation called Opinion Research Services (ORS) to collect the data on their behalf. If you have any questions about this project or if you need help filling in the questionnaire, please contact Claire Thomas at ORS on 01792 535 337 or claire.thomas@ors.org.uk. If you would like to speak to someone from the Department of Health and Social Care, you can contact them at ddu@dh.gsi.gov.uk.
